Payment Policies

Payment of non-surgical treatments such as Botox® Cosmetic and fillers are made at the time of service by credit card; we are unable to accept personal checks for these treatments. Payment for surgery may be made by major credit card or personal check. We also offer patient financing through Care Credit. We do not handle cash at our office, so please be mindful of this policy.

A deposit of $1000 is required to schedule surgery. Full payment for surgery is required four (4) full weeks prior to any scheduled procedure. Purely cosmetic services will not be billed to any third party insurer.

If your surgery is reconstructive in nature, your portion of any visit or service, such as a deductible or co-pay, is paid at the time of service. Please ask specifically about the insurance programs in which Mark Codner, MD is enrolled. For insurance cases in which Dr. Codner is not a preferred provider, payment is required in full at the time of service and is the responsibility of the patient. Our insurance liaison will work as your advocate to assist you through the pre-certification process and help determine the terms of your insurance coverage.

All surgical, cosmetic injection and skin care treatment recommendations are individualized just for you. Therefore, our fees are only provided in person, once you have been evaluated, assessed and appropriate recommendations for treatment are made.
